The study analyzed 572 cases of skin disorders of the external genitalia in men in a Chinese hospital between 2015 and 2025. The average age of the patients was 47.6 years, and the incidence of diseases varied by age: inflammatory conditions were prevalent in adolescents (1.2%), human papillomavirus-related diseases and inflammatory diseases were common in adults (70.6%), and elderly patients (28.1%) were more common tumors. The most frequent diagnoses were inflammatory diseases (33.7%) and tumors (33.4%). Diagnostic discrepancy between clinical assessment and histopathological findings occurred in 34.6% of cases, especially in lichen planus and extramammary Paget's disease. The study highlights that dermoscopy – a non-invasive diagnostic method – improves the accuracy of diagnosis and reduces the need for unnecessary biopsies. The integration of clinical, dermoscopic and histopathological data is essential for the correct treatment of male genital dermatoses.
