The study compared the effectiveness of 5% permethrin, 10% neem leaf extract, and 10% brotowali stem extract for the treatment of scabies in 61 male students. Participants were randomly assigned to three groups and the study took place from April to May 2021. All three treatments significantly reduced the number of skin lesions (p < 0.05), with no significant difference between groups in the median Physician Global Assessment score (p > 0.05). Dermatology Life Quality Index scores improved significantly in all groups, with earlier improvements at day 7 in the neem and brotowali groups. No adverse effects were reported. Neem and brotowali extracts are as effective and safe as permethrin in improving quality of life. Limitations included imperfect monitoring of treatment adherence and the possibility of reinfestation.
