First case report of Shewanella indica isolated from a hospitalized patient in Serbia

also describes the first case of isolation of the bacterium Shewanella indica from the stool of a 72-year-old critically ill man hospitalized in Serbia with multiple comorbidities and co-infections.[1] Shewanella spp. are opportunistic pathogens, especially in patients with malignancies, neutropenia, severe heart failure, renal insufficiency, and hepatobiliary diseases.[1] Initial identification by the VITEK 2 system mislabeled the strain as a Shewanella alga, MALDI-TOF MS was inconclusive, and whole-genome sequencing (WGS) led to definitive determination.[1] WGS revealed multidrug resistance and numerous virulence-associated genes; minimum inhibitory concentrations (MICs) were determined for 32 antibiotics, but interpretation was limited by the absence of species-specific cutoff values.[1] The case highlights the diagnostic difficulties in distinguishing S. indica and the value of genomic methods for accurate identification of rare pathogens in complex clinical situations.[1]
