The study compared the efficacy of dual filter IPL (400–600 nm/800–1200 nm) and red-blue LED phototherapy as an adjunct to systemic doxycycline or isotretinoin for moderate to severe acne in 40 Asian patients. The treatment lasted 4 weeks with a split face: weekly IPL on one side and twice weekly LED on the other. The evaluation was carried out using the Global Acne Grading System (GAGS) and the patient's Likert scale of satisfaction. In the doxycycline group, the mean improvement was 46.08% on the LED side and 48.82% on the IPL side; in the isotretinoin group, 48.62% on the LED side and 46.69% on the IPL side. IPL and LED produced comparable improvement in acne severity with no significant difference (p > 0.05). Patient satisfaction was slightly higher on the IPL side, with "very satisfied" reported by 15% in the doxycycline group and 20% in the isotretinoin group. Adverse reactions (pain, erythema, edema) were transient and more frequent with IPL, without serious events. Both methods showed rapid improvement at the start of treatment.
