The European Center for Disease Prevention and Control (ECDC) has published guidelines on doxycycline post-exposure prophylaxis, known as doxy-PEP, for the prevention of bacterial sexually transmitted infections (STIs). Doxy-PEP involves taking doxycycline as soon as possible after oral, vaginal or anal sex, up to a maximum of 72 hours.[1] ECDC focuses on public health considerations for this method of prevention. Studies show that 200 mg of doxycycline reduces syphilis and chlamydia infections by more than 70% and gonococcal infections by less.[2] It is especially recommended for men who have sex with men and transgender women with a history of syphilis, chlamydia, or gonorrhea in the past 12 months.[1][2] Doxy-PEP should be combined with a comprehensive approach to sexual health, including testing, vaccination and condom use.[1] Guidelines emphasize shared decision-making with the patient about benefits and risks.[2]
