In 2025, the US recorded 2,242 confirmed cases of measles, the highest number in one year since 1991[1]. The first outbreak began in January 2025 in West Texas and spread to several states, resulting in three deaths[1]. In January 2026, there were already 336 confirmed cases, which is more than in the entire years 1993 to 2025[1]. The US will lose measles elimination status if continuous transmission of the same virus strain lasting 12 months or longer is demonstrated[2]. The International Health Organization PAHO will meet on April 13, 2026 to decide whether the US has lost its measles-free designation[1]. Approximately 93% of cases in 2025 occurred in unvaccinated persons or persons with unknown vaccination status[2]. The first child deaths from measles in the last 20 years were recorded in 2025[3].
