It introduces the new GOLD 2026, which brings changes in the treatment of COPD. GOLD 2026 recommends for the first time the active search of patients with COPD, which is primarily the task of general practitioners. Vaccination recommendations are updated, including influenza, COVID-19, pneumococcal, RS virus, DTP, and herpes zoster. In case of influenza, it is also possible to vaccinate during hospitalization in case of acute cardiac decompensation. A fundamental shift has occurred in the classification of the degree of the disease, where one moderately severe exacerbation per year is enough to be classified as category E. A new chapter on COPD and comorbidities has been created, which emphasizes the importance of taking into account extrapulmonary problems such as cardiovascular problems, osteoporosis, sarcopenia, anemia and vitamin D deficiency. A chapter on modern technologies in pneumophthisiology has been added.
