In January 2026, Iranian security forces, including the Islamic Revolutionary Guard Corps, attacked protesters and health workers, violating the principle of medical neutrality protected by international law.[1] The protests broke out on December 28, 2025 and were triggered by economic problems, including the fall of the Iranian rial and rising inflation.[2] Health workers reported unprecedented levels of violence against unarmed protesters, including pregnant women and children, including during attacks on hospitals on January 8 following a nationwide internet blackout.[1] According to various sources, thousands of people have been killed - Iranian authorities have confirmed at least 5,000 casualties, including 500 members of the security forces, while human rights organizations confirm lower figures, warning that the actual number may be higher.[2][3] The international medical community is called upon to show professional solidarity and hold Iran accountable for its deliberate and sustained attacks on patients, health workers and medical facilities.[1]
