The American Medical Association (AMA) recently stated that surgical procedures in minors should generally be delayed until adulthood because evidence of their effectiveness is insufficient.[1] This statement supports the position of the American Society of Plastic Surgeons (ASPS), which recommends delaying breast, genital, and facial gender-affirming surgeries until age 19.[1][3][5] In 2024, the AMA affirmed its policy supporting gender-affirming medicine and surgery based on shared decision-making.[1] There is a consensus among clinicians that the surgeries are mostly performed on adults, although they should be available on a case-by-case basis for minors.[1] The World Professional Association for Transgender Health's 2022 Standards of Care require eight conditions to be met for recommending surgery before adulthood, including diagnostic criteria and addressing mental health.[1] As of January 2025, about a dozen US hospitals have stopped performing surgeries on young transgender people due to federal pressure.[1] The AMA and ASPS announcements come at a time of growing political pressure on transgender care.[1]
