America's disease surveillance system is broken and going dark. The authors propose to build a new system to replace it. The system benefits from detailed and continuous evaluation of data, which enables continuous analysis and targeting of risk groups. The USPSTF's independent group evaluates the effectiveness of screenings based on scientific evidence. Preventive care is covered by private insurance for 60% of the population and government programs such as Medicare and Medicaid. Since 2010, the Affordable Care Act has guaranteed prevention without co-payments. In the USA, prevention is based on data analysis and program optimization.
