The Scandinavian Simvastatin Survival Study more than 30 years ago showed that statins save lives. Despite this, statins are underused worldwide, putting millions of people at risk of cardiovascular events that could be prevented by statins. Persistent safety concerns hinder both prescribing and adherence. Fear of side effects, such as muscle pain, fatigue, sleep disturbances, and perceived cognitive effects, leads to treatment discontinuation. However, randomized nocebo studies have shown that most of these reported symptoms are caused by patient expectation, not the drug itself. Warnings about the safety of statins thus persist despite the evidence.
