Aspiration of chili pepper fragments is an under-recognized cause of respiratory problems, especially in elderly patients. The study describes two cases of elderly people with subacute respiratory symptoms that were initially misdiagnosed as obstructive pneumonia or neoplasia. High-resolution computed tomography revealed characteristic hyperdense, V-shaped, or ring-like masses in the bronchial structures that suggested foreign body inhalation. Flexible bronchoscopy confirmed the presence of chili pepper fragments clogging the airways. Cryoextraction successfully removed samples without fragmenting them. After treatment, symptoms improved significantly, although residual bronchiectasis remained. The study highlights the need for increased clinical awareness of this etiology and prompt intervention to prevent complications.
