An extensive review of the evidence suggests that exercise therapy for osteoarthritis offers little and short-term pain relief. The analysis included five reviews with 8631 participants and 28 additional studies with 4360 participants, a total of more than 13,000 people. For knee osteoarthritis pain, the effects were small and short-lived compared to placebo (mean difference -10.8, 95% CI -19.1 to -2.6) and no treatment (-12.4, 95% CI -15.6 to -9.2), but the certainty of the evidence was very low. In larger or longer-term studies, the effects diminished. For hip osteoarthritis, the effects were negligible (-6.7, 95% CI -9.3 to -4.0) with moderate certainty of evidence and for hands small (-10.0, 95% CI -15.5 to -4.5). The results were comparable to education, manual therapy, analgesics, injections or arthroscopy, but less effective than osteotomy or joint replacement in the long term.[1]
