A 36-year-old woman after multiple births had intermittent mild bleeding menstrual periods without abdominal pain or painful menstruation for 1 year after the insertion of an intrauterine device (IUD). Imaging studies, including X-ray and pelvic ultrasound, revealed complete perforation of the uterus and displacement of the IUD into the pelvic cavity. The IUD was removed by a combined hysteroscopic and laparoscopic procedure. Complications associated with IUDs, such as perforation and displacement, can only be manifested by abnormal uterine bleeding. Ultrasonography is the gold standard for evaluating these complications. Early diagnosis is important to prevent long-term consequences. With persistent irregular bleeding, patients should be vigilant even without pain.
