This systematic review investigated the association between dry eye disease (DED) and personality traits. According to PRISMA guidelines, PubMed, Scopus and Web of Science were searched in November 2025, identifying 408 records but including 8 studies. Four studies had a clinically confirmed diagnosis of DED and four relied on self-reported symptoms. In both groups, personality traits, especially neuroticism and harm avoidance, were associated with greater symptom burden and reduced quality of life. Regression and mediation analyzes in several studies confirmed the influence of personality on the perception of symptoms. Findings show that personality traits influence the subjective experience and clinical impact of DED. However, cross-sectional designs and self-reported measures limit causal interpretation. Further longitudinal and multimethod research is needed to elucidate mechanisms and develop integrated clinical approaches.
