Patients in the US may switch to self-managing their warfarin, according to a small feasibility study. Among 138 patients who agreed to make their own decisions about warfarin dosing based on home monitoring, 87% successfully completed this task. The study suggests that patients can self-administer warfarin without problems. Warfarin is a medicine used to prevent blood clots. Its dosage requires regular monitoring such as an INR of 2.00 to 3.00. Patients should be monitored for signs of bleeding, such as bruising, blood from the nose or in the stool.
