A scoping systematic review published March 23 in JAMA Network Open examined the promotion of prescription drugs by social media influencers. Promotion has been associated with misinformation, limited regulatory oversight, and the use of personal stories that obscure advertising intent. Influencers often present health information outside of their clinical expertise, exaggerate benefits or omit alternative treatments. One analysis showed that on sexual health topics, they shared inaccurate information about contraception and prevention of sexually transmitted infections despite high confidence in knowledge. Motivated by peer support, patient influencers blur the line between personal experience and advertising. Personal illness stories in sponsored posts reduce the recognition of persuasive intent and lead to more favorable attitudes toward promoted drugs. The review highlights the need for better regulatory guidance and standardized sponsorship tagging rules such as #ad or #sponsored hashtags, but these are inconsistent.
