Diabetes patients are dying in hospital because of insulin mismanagement, the Health Services Safety Investigations Body (HSSIB) has warned. Insulin-dependent patients face persistent and serious safety risks when admitted to hospital. Insulin is often given after a meal instead of before it, and sometimes too high doses are given. Some patients have harmed themselves because they were not allowed to self-manage their diabetes and administer their insulin. In addition, they removed their wearable diabetes monitoring technology during their hospital stay. Doctors in these cases did not manage the condition of the patients effectively. The HSSIB emphasizes that insulin is a high-risk drug and is among the most common causes of drug-induced harm.
