Macrophage-targeted glucocorticoid prodrug resolves acute inflammation while preserving HPA axis function: mechanistic, preclinical, and Phase II/III clinical evidence

Researchers developed a novel drug 101-PGC-005, a modified form of dexamethasone targeted to macrophages via the CD206 receptor. Unlike conventional glucocorticoids, this drug is rapidly cleared from the body and does not impair normal hypothalamic-pituitary-adrenal (HPA) axis function. In a clinical trial with 309 hospitalized COVID-19 patients, 101-PGC-005 was superior to standard dexamethasone—patients improved by 2 points on the WHO scale with a median of 3 days compared to 4 days in the control group. The drug was also superior in viral clearance, oxygen saturation improvement, and fever resolution. The safety profile was comparable in both groups with no serious adverse events. The mechanism of action involves selective elimination of chronically activated macrophages that sustain inflammation while preserving natural antiviral signaling and HPA axis function.
