Traumatic intracranial haematoma often presents as a neurosurgical emergency. Timely surgical evacuation can be life-saving and prevent patient deterioration. Nevertheless, some patients may develop severe brain swelling after haematoma evacuation. This swelling leads to brain compression and neurological deterioration. In The Lancet Neurology, Junfeng Feng and colleagues reported a randomised controlled trial. The study focused on the safety and efficacy of additive decompressive craniectomy. It was intended for patients with a large acute traumatic epidural haematoma with signs of tentorial herniation.
