The article describes a rare case of complete lens dislocation into the vitreous cavity in a 90-year-old female patient who sustained blunt ocular trauma. Examinations revealed an intact anterior capsule with posterior capsule rupture, complete lens dislocation, and zonular dehiscence. The patient underwent pars plana vitrectomy, lensectomy, and scleral-fixated intraocular lens implantation. Postoperatively, visual acuity improved gradually, intraocular pressure remained stable, and the lens implant was well-centered. This complication is extremely rare and can be easily missed in clinical practice. The authors emphasize the importance of multimodal imaging for diagnosis and individualized surgical management, particularly in elderly patients.
