The article presents a new blood pressure measurement method called Diffuse Speckle Pulsatile Flowmetry (DSPF) that does not require an inflatable cuff. This laser-based technique simultaneously records blood flow rate and blood volume from deeper vascular tissues. In an outpatient study with 133 patients, the method achieved mean absolute errors of 4.17 mmHg for systolic pressure and 2.42 mmHg for diastolic pressure compared to standard methods. The method met the requirements of the international AAMI/ESH/ISO 81060-2:2018 standard and received the highest grade "A" according to the British Hypertension Society. In an inpatient study with two patients, errors in measuring pulse waves were 2.35 and 3.06 mmHg compared to arterial line measurements. DSPF demonstrated better results than traditional photoplethysmography and can be used in outpatient, inpatient, and remote monitoring settings.
