Reducing medication burden and improving pressure injury outcomes in older emergency patients: a pharmacist-nurse collaboration

The study examined the impact of pharmacist-nurse collaboration on pressure injury treatment in older emergency patients. A total of 165 patients were studied before program implementation and 175 patients after implementation. The program included medication review, treatment regimen optimization, risk-stratified care, patient repositioning, and nutritional support. Following implementation, the average number of prescribed medications decreased by 3.37, treatment regimen complexity declined significantly, and medication costs were reduced. Patients in the post-implementation group showed higher rates of clinical recovery (10.3% versus 1.2%), pressure injury improvement (87.4% versus 77.0%), and Braden score improvement (78.9% versus 53.9%). The program proved to be an effective and practically feasible approach to improve pressure injury management.
