Early Biomarkers as Predictors of Nephrology Trial Trajectory

Chronic kidney diseases represent a significant global health challenge with rising prevalence and unmet medical needs. Nephrology clinical trials are time-intensive, with Phase III studies averaging 34.7 months duration and enrollment periods taking 21.9 months on average. Trial costs are substantial, with Phase II studies costing an average of $20.57 million and Phase III studies $29.82 million. Biomarkers are biological indicators that predict the likelihood of therapeutic benefit or harm from a drug, enabling sponsors to anticipate outcomes during treatment. Biomarkers have been used in approximately 47 percent of all nephrology trials, with highest usage in Phases II and III. The majority of biomarkers deployed in nephrology trials have been used for evaluating efficacy, followed by patient screening and stratification, and then safety monitoring.
