The RACOVIR study was an observational descriptive study aimed at evaluating the management of patients with non-valvular atrial fibrillation (NVAF) receiving oral anticoagulants in primary care in Spain. A total of 1,901 patients were included: 428 received vitamin K antagonists (VKA) and 1,473 received direct oral anticoagulants (DOAC). Patients treated with VKA were significantly older and had higher prevalence of hypertension. Mean treatment duration was 7.6 years for VKA and 3.8 years for DOAC. Among VKA patients, 56.2% to 59.0% achieved good anticoagulation control according to different methods. Patients in the DOAC group reported greater satisfaction with treatment, including better perception of benefits and lower impact on daily life. The incidence of stroke was higher with DOAC (1.75 per 1,000 person-years) compared to VKA, as was the incidence of major bleeding (3.68 per 1,000 person-years).
