A new profession of care coordinators is being introduced in Czech healthcare, who will work in centers treating stroke patients. The stroke care coordinator has a minimum workload of 1.0 in complex cerebrovascular centers and 0.5 workload in stroke centers. The coordinator's main task is to accompany the patient immediately after a stroke, especially during the first three months, and ensure that the patient undergoes all recommended examinations and follow-ups. The coordinator verifies whether the patient has undergone tests to determine the cause of the stroke, such as ECG, heart rhythm monitoring, or echocardiography, and helps arrange them. They also monitor the continuity of further treatment including physiotherapy, speech therapy, and psychological care. After three months, the coordinator assesses the patient's functional status using the modified Rankin Scale and enters the data into a quality registry. Unlike an oncology coordinator who helps the patient reach the center, a stroke coordinator helps the patient safely continue care after leaving the hospital.
