Reduced option generation reveals distinct profiles of apathy in schizophrenia

The study examined the ability of patients with schizophrenia to generate action options using an option generation task (OGT) in a sample of 150 patients at various stages of schizophrenia and 100 healthy controls from the UK, Spain, and China. Patients generated the same number of paths as controls but their options were approximately half as unique. Reduced uniqueness of generated options was negatively associated with patients' self-reported behavioral apathy. Analysis revealed four cognitive mechanisms (roaming entropy, option elaboration, perseveration, and novelty maintenance) that explained 69 percent of the variance in uniqueness. K-means clustering identified three distinct patient groups with different clinical signatures: a small group performing similarly to controls, a larger group with impaired option development and depressive symptoms, and a third group with broadly reduced exploration. The findings suggest that reduced generation of unique action possibilities in apathy may be a remediable component.
