Efficacy and safety of [161Tb]Tb-PSMA radioligand therapy in metastatic castration resistant prostate cancer: a systematic review and meta-analysis

This study evaluated the efficacy and safety of a novel radioligand therapy with Terbium-161 ([161Tb]Tb-PSMA) in patients with metastatic castration-resistant prostate cancer. The analysis included six studies from 2024-2026 with a total of 80 patients who underwent 291 treatment cycles. Most patients had previously received treatment and were already treated with other PSMA-targeted therapies. Results showed disease control in 83.9% of patients and objective response to treatment in 50.2% of patients. Overall, 124 adverse events were reported, with only 18 being high-grade. The authors conclude that [161Tb]Tb-PSMA therapy is a promising treatment option with an acceptable safety profile, although larger and longer-term clinical trials are needed.
