Kratom represents only a marginal problem in addiction medicine practice, with clients whose primary issue is kratom use encountered only sporadically in individual cases. Regular or long-term kratom use can lead to dependence, tolerance development, and withdrawal symptoms such as restlessness, insomnia, anxiety, or irritability. Combining kratom with other opioids or alcohol is risky due to potentiation of depressant effects and risk of respiratory depression. According to experts, greater attention should be paid to alcohol, which represents the most common reason for seeking care in addiction clinics and is associated with significantly higher numbers of health, psychological, and social impacts. Raising the age limit from 18 to 21 years and stricter quality laboratory controls could reduce the number of new users and contamination cases. However, overly strict regulation may lead to market shift to illegality and substitution with other psychoactive substances carrying greater health risks.
