Community-Based Screening for Chronic Kidney Disease

This project focused on community-based screening for chronic kidney disease. The main goal was to identify and address this disease within the community. Monitored conditions included chronic kidney disease and hypertension. Interventions involved providing kidney-protective care in outpatient clinics. The project was sponsored by the University of Texas at Austin. It has been successfully completed.
