The article addresses perioperative management of patients taking sodium-glucose cotransporter 2 inhibitors (SGLT2i). The multidisciplinary consensus statement from the Society for Perioperative Assessment and Quality Improvement (SPAQI) by Oprea and colleagues represents an important step in weighing the risks and benefits of perioperative care for these patients. SGLT2i have well-established benefits including improved glycaemic control, reduced cardiovascular morbidity and mortality, and improved renal function. The main risk associated with SGLT2i use is euglycaemic diabetic ketoacidosis (eDKA). The statement aims to balance these proven benefits against potential risks during the perioperative period.
