The Czech healthcare system evaluates new medicines primarily based on QALY and ICER indicators, which measure health benefits and cost-effectiveness. However, this traditional approach does not account for broader societal impacts of treatment, such as patients returning to work, reduced need for informal care, or family impact. Another problem is that the same QALY has equal value regardless of disease severity. Value-based pricing should better capture these broader benefits, but the Czech system is historically built primarily on the perspective of public health insurance. Since 2022, societal aspects have been considered for rare disease medicines, and since January 2024, also for vaccines and monoclonal antibodies for prophylaxis. Data on social security costs are useful for argumentation, but their influence on final decisions remains limited.
