Case Report: Nebulized autologous platelet-rich plasma associated with quantitative CT and clinical changes in a patient with COPD and chronic hypersensitivity pneumonitis

This case report describes a 70-year-old man with a long-standing history of COPD and chronic hypersensitivity pneumonitis who underwent eight treatment cycles with nebulized autologous platelet-rich plasma (PRP) at approximately 4-week intervals. Changes during treatment were assessed using serial CT with quantitative analysis, pulmonary function tests, and patient-reported outcomes. Quantitative CT demonstrated a reduction in low attenuation area percentage from 17.4% to 13.6%, representing a 22% relative reduction, along with an 11.5% decrease in total inspiratory lung volume. Formal spirometry showed an approximately 20% increase in peak expiratory flow compared to pre-treatment baseline. The patient reported marked reductions in exertional dyspnea, substantially improved walking tolerance, and increased energy levels. Nebulized PRP was well tolerated with no reported adverse events.
