The study examined 534 patients with minor ischemic stroke (low NIHSS score 0-5) who underwent multiparametric CT imaging. Patients were classified into three groups based on the presence of perfusion deficits and vessel occlusion. Early neurological deterioration occurred in 8.1% of cases, most frequently in the group with perfusion deficit and vessel occlusion (17.1%). Disability at 90 days (mRS ≥3) occurred in 17.6% of cases, most commonly in the group with perfusion deficit without visible occlusion (34.4%). The combination of perfusion deficit and vessel occlusion was independently associated with early neurological deterioration, while perfusion deficit without occlusion was associated with 90-day disability. Multiparametric CT imaging may help identify patients at higher risk of adverse outcomes.
