Potential five-year life years associated with stage IV reductions in NHS Galleri Trial

The NHS-Galleri trial did not achieve its primary endpoint of reducing combined stage III and IV cases, but reported fewer stage IV cancer diagnoses after annual multi-cancer early detection screening. Researchers analyzed potential five-year life-years associated with stage IV differences observed during two incident screening rounds. During the incident rounds, the intervention arm had 180 stage IV cases and the control arm had 239 cases. The projection showed 66.3 potential life-years over five years in the intervention cohort, equivalent to 466 per million people screened annually (95% interval 190-743). Colorectal, lung, and esophageal cancers accounted for 79% of the net projection. If the observed reduction in stage IV diagnoses represents true downstaging to stage III, it could produce meaningful population health gains. Longer follow-up remains necessary to distinguish true benefit from chance, lead-time and length biases, overdiagnosis, and differences in tumor biology.
